RESUMO

Black esophagus (BE) or Acute esophageal necrosis (AEN) is rare clinical entity with an estimated incidence of 0.01-0.2% of patiens undergoing esophagogastroduodenoscopy (EGD). BE is characterized by circumferential blackish coloration of the mucosa, with abrupt interruption at the esophagogastric junction level, secondary to hypoperfusion and ischemia. Upper gastrointestinal hemorrhage is the most common presentation (70-90%), being a rare cause of it. We present 3 cases that were admitted in our center for hematemesis.

RPE65 retinol isomerase is an indispensable player in the visual cycle between the vertebrate retina and RPE. Although membrane association is critical for RPE65 function, its mechanism is not clear. Residues 107-125 are believed to interact with membranes but are unresolved in all RPE65 crystal structures, whereas palmitoylation at C112 also plays a role. We report the mechanism of membrane recognition and binding by RPE65. Binding of aa107-125 synthetic peptide with membrane-mimicking micellar surfaces induces transition from unstructured loop to amphipathic α-helical (AH) structure but this transition is automatic in the C112-palmitoylated peptide. We demonstrate that the AH significantly affects palmitoylation level, membrane association, and isomerization activity of RPE65. Furthermore, aa107-125 functions as a membrane sensor and the AH as a membrane-targeting motif. Molecular dynamic simulations clearly show AH-membrane insertion, supporting our experimental findings. Collectively, these studies allow us to propose a working model for RPE65-membrane binding, and to provide a novel role for cysteine palmitoylation.

Introducción y objetivoEl exceso de peso puede inducir modificaciones en la estructura y función del miocardio. La presencia de hipertrofia ventricular izquierda es un predictor independiente de morbimortalidad cardiovascular.El objetivo principal del estudio ha sido conocer la prevalencia de alteraciones morfofuncionales cardiacas en pacientes con obesidad y su modificación tras la pérdida de peso después de una cirugía bariátrica (CB).Pacientes y métodosEstudio de cohortes prospectivo de 75 pacientes con obesidad y sin cardiopatía conocida a los que se les realizó un bypass gástrico. Se midieron parámetros antropométricos, analíticos y ecocardiográficos antes, a los 6 y 12 meses de la intervención.ResultadosSe incluyeron 75 pacientes (66,6% mujeres, edad media 39,3 [9,7] años e índice de masa corporal [IMC] 47,8 [7,1] kg/m2). A los 6 y 12 meses de la CB se produjo una reducción significativa del peso corporal, una mejora en los parámetros metabólicos, inflamatorios y protrombóticos, así como en los factores de riesgo cardiovascular asociados a la obesidad (hipertensión arterial [HTA], diabetes mellitus tipo 2 [DM2], dislipemia [DLP] y síndrome de apnea-hipopnea del sueño [SAHOS]).Antes de la intervención, el 62,7% de los pacientes presentaba alteración en la geometría del ventrículo izquierdo, siendo el remodelado concéntrico la más frecuente (38,7%). Además, el 50,7% presentaba disfunción diastólica. Al año de la CB, el patrón ventricular fue normal en el 92% de los casos y la función diastólica mejoró significativamente.ConclusionesNuestros resultados corroboran el efecto negativo de la obesidad sobre el miocardio, así como la potencial reversibilidad de estas alteraciones tras una pérdida significativa de peso después de una CB. Diastolic dysfunction occurred in 50.7% of the patients.One year after surgery, the ventricular pattern was normal in 92% of cases and the diastolic function improved significantly.ConclusionsOur results support the negative effect of obesity on cardiac geometry and function and the potential reversibility of these cardiac alterations after marked weight loss due to BS. RESUMEN La atención odontológica requiere del cumplimiento de todas las normas legales referente al protocolo de atención, ya que tanto el profesional como el paciente deben estar protegidos previendo cualquier conflicto que pudiera surgir. El objetivo de este trabajo fue evaluar el nivel de conocimiento sobre deberes y obligaciones en la práctica odontológica en el Paraguay. El estudio fue observacional descriptivo de corte transversal, con una muestra total de 100 odontólogos del Hospital Central de Policía Rigoberto Caballero. El cuestionario constó de 20 preguntas de las cuales 4 fueron sobre características sociodemográficas y académicas, 13 preguntas de conocimiento sobre deberes y obligaciones en la práctica odontológica, 1 sobre capacitación recibida en legislación, 1 sobre cumplimiento de obligaciones tributarias y 1 sobre barreras de protección usadas contra rayos X. El nivel de conocimiento fue clasificado teniendo en cuenta el porcentaje de respuestas correctas: insuficiente (<60%) y suficiente (≥60%). Ochenta por ciento de los encuestados fueron del sexo femenino con edad promedio de 40 años. El nivel de conocimiento general sobre deberes y obligaciones que rigen la práctica odontológica fue suficiente en el 71% de los odontólogos encuestados. No se encontraron diferencias significativas entre el nivel de conocimiento según la universidad (p=0,21) y año de egreso (p=0,34). El nivel de conocimiento sobre deberes y obligaciones que rigen la práctica odontológica en el Paraguay fue suficiente, sin embargo, algunos aspectos específicos como conocimiento tributario, documentaciones que rigen las leyes, recibo de comisiones y notificaciones de enfermedades transmisibles deben ser todavía más explorados, estudiados y difundidos.


ABSTRACT Dental care requires compliance with all legal regulations regarding the care protocol, since both the professional and the patient must be protected anticipating any conflict that may arise. The objective of this study was to evaluate the level of knowledge about duties and obligations in dental practice in Paraguay. The study was descriptive, cross-sectional, observational, with a total sample of 100 dentists from the Rigoberto Caballero Central Police Hospital. The questionnaire had 20 questions of which 4 were about knowledge on sociodemographic and academic characteristics, 13 on duties and obligations in dental practice, 1 on training received on legislation, 1 on compliance with tax obligations and 1 on barriers used for protection againts X-ray. The level of knowledge was classified considering the percentage of correct answers: insufficient (<60%) and sufficient (≥60%). Eighty percent of those surveyed were female with an average age of 40 years. The level of general knowledge about duties and obligations that govern dental practice was sufficient in 71% of the surveyed dentists. No significant differences were found between the level of knowledge according to university (p = 0.21) and year of graduation (p = 0.34). The level of knowledge about duties and obligations that govern dental practice in Paraguay was sufficient, however, some specific aspects such as tax knowledge, documentations that govern the laws, receipt of commissions and notifications of communicable diseases must be even more explored, studied and disseminated.

Objective: To evaluate the economic and psychological repercussions, among other difficulties encountered during the COVID-19 pandemic in a group of dentists. Materials and methods: Cross-sectional descriptive observational study. An online questionnaire was applied to dentists from the "Rigoberto Caballero" Central Police Hospital (HCPRC) of Asunción-Paraguay that was distributed between July and September 2021. It consisted of 32 closed questions distributed in 5 sections: sociodemographic data, economic effects of the pandemic, psychological effects, contagion, and experiences with respect to the clinic. The data were analyzed with the Epi Info version 7 program and the results are represented in tables, the Fisher's exact test and the Chi-square test were also used. Results: 78 dentists participated, 78.2% female, 56.4% from the 35-45 age group. 88.5% reported a decrease in income in private practice and the majority had to resort to another source to be able to cover daily expenses. Most reported having suffered symptoms of anxiety and depression during the pandemic and felt the need to consult a psychiatrist. It was reported an increase in the demand for Personal Protective Equipment (PPE) and an increase in its costs. Almost half of all dentists reported getting COVID-19. Conclusion: The pandemic had a considerable impact on HCPRC dentists, causing financial problems, psychological repercussions and difficulties with prices and a shortage of PPE.

BACKGROUND: The relationship between asthma and vitamin D deficiency has been known for some time. However, interventional studies conducted in this regard have shown conflicting results. OBJECTIVE: To evaluate the efficacy of vitamin D supplementation in asthmatic patients in improving the degree of control of asthma. METHODS: Randomised, triple-blind, placebo-controlled, parallel-group study in adult asthmatic patients with serum 25-hydroxyvitamin-D3 <30 ng/mL. The intervention group received oral supplementation with 16 000 IU of calcifediol per week, and the control group had placebo added to their usual asthma treatment. The study period was 6 months. The primary endpoint was the degree of asthma control as determined by the asthma control test (ACT). Secondary endpoints included quality of life measured using the mini Asthma Quality of Life Questionnaire, the number of asthma attacks, oral corticosteroid cycles, the dose of inhaled corticosteroids, number of emergency visits, unscheduled consultations with the primary care physician and hospitalisations for asthma. RESULTS: One hundred and twelve patients were randomised (mean age 55 years, with 87 (78%) being women). Of the 112 patients, 106 (95%) completed the trial. Half the patients (56) were assigned to the intervention group and the other half to the control group. A statistically significant clinical improvement was observed in the intervention group (+3.09) compared with the control group (-0.57) (difference 3.66 (95% CI 0.89 to 5.43); p<0.001) as measured using ACT scores. Among the secondary endpoints, a significant improvement in the quality of life was found in the intervention group (5.34), compared with the control group (4.64) (difference 0.7 (95% CI 0.15 to 1.25); p=0.01). CONCLUSION: Among adults with asthma and vitamin D deficiency, supplementation with weekly oral calcifediol compared with placebo improved asthma control over 6 months. Further research is needed to assess long-term efficacy and safety. TRIAL REGISTRATION NUMBER: NCT02805907.

El uso descontrolado de los antibióticos favoreció el desarrollo de resistencia bacteriana que va aumentando considerablemente conforme van pasando los años y se ha convertido en una de las mayores amenazas para la salud pública. Es por ello que el Ministerio de Salud Pública y Bienestar Social (MSPyBS) emitió la Resolución S.G. N° 576/18 en la que se dispone la venta obligatoria de medicamentos que tengan actividad antibiótica de uso sistémico bajo receta simple archivada. El objetivo de este trabajo fue medir el nivel de conocimiento y práctica de la normativa de la receta simple archivada para la prescripción de antibióticos de uso sistémico en odontólogos de Asunción y Gran Asunción. El diseño del estudio fue observacional descriptivo, de corte transversal, en donde participaron 190 profesionales odontólogos. Los resultados obtenidos en este estudio mostraron que el 77,89% de los odontólogos respondió que conoce la Resolución S.G. N° 576/18 del MSPyBS, y 94,21% respondió que sabe qué datos incluir en la receta simple archivada. Sólo 11,28% incluye correctamente todos los datos requeridos. También se pudo observar que la amoxicilina es el antibiótico más recetado por los odontólogos (79,47%). En conclusión, se observó que solo el 40,0% de los profesionales encuestados tuvieron un nivel de conocimiento suficiente, mientras que el 60,52% tuvo un nivel de práctica adecuado


The uncontrolled use of antibiotics favored the development of bacterial resistance that has increased considerably in the last years and has become one of the greatest threats to public health. This is the reason why the Ministry of Public Health and Social Welfare (MSPyBS) issued the Resolution SG No. 576/18, which stipulates the mandatory sale of drugs that have systemic antibiotic activity under a simple archived prescription. The aim of this study was to measure the level of knowledge and practice on the regulation of the simple archived prescription for systemic antibiotics of dentists in Asunción and Gran Asunción. The study design was descriptive, cross-sectional and 190 dentists participated. The results showed that 77.89% of dentists knew about Resolution S.G. No. 576/18 of the MSPyBS, and 94.21% replied that they knew what information to include in the simple archived prescription. Only 11.28% correctly included all the required data. It was also observed that amoxicillin was the most prescribed antibiotic by dentists (79.47%). In conclusion, it was observed that only 40.0% of the professionals surveyed had a sufficient level of knowledge, while 60.52% had an adequate level of practice

Introducción: Los pacientes con trastornos mentales están sometidos a un mayor número de factores de riesgo de enfermedades bucodentales y temporomandibulares. Ello debido a los efectos secundarios de las medicaciones que consumen, la falta de autocuidado, la dificultad para acceder a atención, la actitud hacia los profesionales sanitarios y también la falta de cooperación en los tratamientos dentales. Objetivo: Determinar la frecuencia de los trastornos temporomandibulares (TTM) y el índice de dientes cariados, perdidos y obturados (CPO-D) en pacientes con discapacidad psicosocial crónica internados en el Hospital Psiquiátrico de la ciudad de Asunción, Paraguay. Material y Métodos: El estudio tuvo un diseño observacional descriptivo de corte transversal y fue aprobado por el comité de ética institucional. Participaron 139 personas con discapacidad psicosocial crónica de ambos sexos, mayores de 18 años, en el mes de mayo, 2018. Como instrumentos de medición se utilizaron: un cuestionario de 3 preguntas, el examen clínico con 5 indicadores para la variable presencia de TTM, el odontograma para el índice CPO-D y los expedientes clínicos. Resultados: Se encontró una población en su mayoría masculina, con un rango de edad entre 40-50 años, y un alto porcentaje de fumadores activos. Se observó que el 83,5 % de los examinados presentaba TTM. El índice CPO-D fue 22,8. Entre los signos de TTM, el ruido articular fue el más común. Conclusión: Se observó una elevada frecuencia de TTM en las personas con discapacidad psicosocial y un elevado índice CPO-D. Introduction: Patients with mental disorders are subject to a greater number of risk factors for oral and temporomandibular diseases. This is due to the side effects of the medication they consume, the lack of self-care, the difficulty in accessing medical help, the attitude of the patients towards health professionals and also the lack of cooperation in their dental treatments. Objective: To determined the frequency of temporomandibular disorders (TMD) and the decayed, missing and filled teeth index (DMFT) in people with chronic psychosocial disability admitted to the Psychiatric Hospital of the city of Asunción. Material and Methods: This descriptive, observational and cross-sectional study was approved by the local ethics committee. One hundred and thirty-nine people with chronic psychosocial disabilities, of both sexes, over 18 years of age participated during May, 2018. The instruments used were a 3-item questionnaire, clinical examination with 5 indicators for presence of TMD, the odontogram for the DMFT index and the clinical charts. Results:A predominantly male population, with an age range between 40-50 years, and a high percentage of active smokers was found. TMD was found in 83.5% and the DMFT index was 22.8. Among the signs of TMD, joint noise was the most common. Conclusion:A high frequency of TMD was observed in people with psychosocial disabilities and a high CPO-D index. Human functional spinal unit (FSU) is comprised of two adjacent vertebrae, the intervertebral disc (IVD), the cartilage endplates and the connecting ligamentous tissues. As humans age, trabecular, cortical bone and IVD change in mechanical properties. These changes influence the movement of the FSU (displacements, rotations and disc bulges). This paper proposes a method to determine the influence of sex, age, weight and height on the movements of the FSU using a combination of the FEM and RSM. The work concentrates on the medium-sized human FSU (L3-L4 lumbar level with standard dimensions of IVD: width = 50 mm, depth = 35 mm and height = 11 mm). Also, the proposed method could be an improvement to the determination of risk of damage based on body mass index (BMI), as well as an alternative method of searching for a healthy weight. The method was developed as follows: First, a three-dimensional parameterized human healthy FSU FE model was generated based on the standard test (for compression, flexion, extension, lateral bending, torsion and shear). The movements of the FSU FE model based on the aforementioned standardized test when the sex, age, weight and height varies in a range for each individual (30-80 years; 70-120 kg; 160-190 cm) were simulated. Quadratic regression models were generated for each standardized test using RSM. An ANOVA test showed that all of the tests have a significant influence on weight and age, whereas height influences only those tests that involve rotation of the FSU (flexion, extension and lateral bending). Normal weight, overweight and obesity BMI classifications were considered in determining the healthy weight with the following constraints: the limit angles of flexion and lateral bending, as well as the maximum posterior bulge limit of the IVD, could not exceed of one-third of the spinal canal. The proposed method concluded that: (1) normal BMI values do not cause damage to the FSU, (2) some overweight BMI values may cause damage to the FSU for a certain range of ages and heights and (3) for BMI obesity values, the risk of damage to the FSU may be imminent.

BACKGROUND: Finite element analysis was used to compare fixation methods for double pelvic osteotomy (DPO). Using 3D scanning a stereolithography (stl) image was produced of a canine pelvis and this was subsequently refined in computer aided design (CAD). Using the CAD files, the images were imported in MSC Marc software to produce a working finite element (FE) model with 3 dimensional tetrahedral elements with linear shaped functions. The dimensions of a precontoured pelvic osteotomy plate with eight screws and a twisted seven screw straight plate were used to build the 2 fixations implants for the FE models. An equivalent load of 300 N was applied progressively on all FE models in order to facilitate its convergence. The load was applied in a distributed manner on the femur-hip joint contact area in order to simulate the actual behavior of the joint. The aim of the present study was to analyze the difference in stiffness and behavior under loading between a lateral vs ventral plate fixation, with unlocked screws and different gap scenarios, for stabilization of a pelvic osteotomy using finite element analysis. RESULTS: From both configurations the maximum displacement of the ventral plate with 7 screws without gap had a value of 1.988 mm, while in the DPO plate had a maximum displacement of 2.191 mm. The load applied for each of the different configurations studied when a gap of 1° was considered and also when a condition of no gap was considered. The ventral plate was stiffer than the lateral plate when a gap was not present. When the gap was closed in the ventral plate, the stiffness increased until a point that remained constant. CONCLUSIONS: Ventral plate fixation can be as or more stiff as lateral plate fixation and provides flexible fixation. This behavior should reduce screw loosening. Using ventral plate fixation is recommended to reduce screw loosening or failure.

BACKGROUND AND OBJECTIVES: Resistin was originally suggested to be a potential mediator of obesity-related insulin resistance in rodents. However, in humans, the role of resistin in obesity and insulin resistance has not yet been demonstrated. The present study investigates whether there are differences in resistin levels between patients with morbid obesity and lean subjects, and analyzes changes in resistin levels after significant weight loss secondary to bariatric surgery. METHODS: Sixty-eight patients with morbid obesity (body mass index [BMI] ≥ 40 kg/m2) and 31 lean subjects (BMI < 25 kg/m2) were selected. The study variables were: weight, height, BMI, waist-hip ratio (WHR), fat mass, family history of cardiovascular disease (CVD), type 2 diabetes mellitus (DM), hypertension, dyslipidemia, smoking, glucose, glycated hemoglobin (HbA1c), insulin, high-sensitivity C-reactive protein (hsCRP), interleukin-6 (IL-6), adiponectin and resistin. Homoeostasis model assessment (HOMA) and quantitative insulin sensitivity check index (QUICKI) were calculated. The obese patients underwent gastric bypass surgery, and the above mentioned variables were reassessed after 12 months and major weight loss. RESULTS: There were no significant differences in resistin levels between morbidly obese patients and healthy subjects of normal weight, or between obese patients before and after weight loss. Resistin levels in morbidly obese patients were not correlated to adiposity anthropometric measures, insulin, glucose, HOMA, QUICKI, hsCRP, IL-6 or adiponectin. In the morbid obesity group, after one year of weight loss, the only study parameter correlated to resistin levels was IL-6. CONCLUSION: Our results do not support a relationship among resistin levels, obesity and insulin resistance in humans.

Introducción y objetivos: inicialmente se sugirió que la resistina era un mediador potencial de la resistencia a la insulina relacionada con la obesidad en roedores. Sin embargo, en seres humanos, el papel de la resistina en la obesidad y la resistencia a la insulina aún no se ha demostrado. El presente estudio investiga si existen diferencias en los niveles de resistina entre pacientes con obesidad mórbida y sujetos con normopeso, y analiza los cambios en los niveles de resistina después de la pérdida significativa de peso debida a cirugía bariátrica. Métodos: se seleccionaron 68 pacientes con obesidad mórbida (IMC ≥ 40 kg/m2) y 31 sujetos normopeso (IMC < 25 kg/m2). Las variables del estudio fueron peso, talla, IMC, relación cintura-cadera (WHR), masa grasa, antecedentes familiares de enfermedad cardiovascular, diabetes mellitus tipo 2 (DM), hipertensión arterial, dislipidemia, tabaquismo, glucosa, hemoglobina glicosilada (HbA1c), insulina, proteína C reactiva de alta sensibilidad (hsCRP), interleucina-6 (IL-6), adiponectina y resistina. Se calcularon la evaluación del modelo de homeostasis (HOMA) y el índice cuantitativo de control de sensibilidad a la insulina (QUICKI). Los pacientes obesos se sometieron a un bypass gástrico, y las variables mencionadas fueron reevaluadas después de 12 meses y una pérdida de peso importante. Resultados: no hubo diferencias significativas en los niveles de resistina entre pacientes obesos mórbidos y sujetos sanos de peso normal, ni entre pacientes obesos antes y después de la pérdida de peso. Los niveles de resistina en pacientes obesos mórbidos no se correlacionaron con medidas antropométricas de adiposidad, insulina, glucosa, HOMA, QUICKI, hsCRP, IL-6 o adiponectina. En el grupo de obesos mórbidos, al año de la pérdida de peso experimentada, el único parámetro del estudio correlacionado con los niveles de resistina fue la IL-6. Conclusión: nuestros resultados no apoyan una relación entre los niveles de resistina, la obesidad y la resistencia a la insulina en los seres humanos (AU)
